POSITION DETAILS
The Journey Coordinator plays a pivotal role within the MyndKind service system, acting as an independent partner in facilitating an individual’s care journey. This position may function as a standalone service, be embedded within a Front Door service, or complement other MyndKind offerings. The Journey Coordinator offers person-centred practice, ensuring individuals are at the core of their health and wellbeing. Through collaborative care coordination, the role aims to enhance service integration, improve access to appropriate supports, and promote positive mental health outcomes.

RESPONSIBILITIES
Participant Support
* Use the IAR-DST (Intake and Assessment Referral – Decision Support Tool) to assess eligibility for Journey Coordination and refer participants to other MyndKind services where appropriate.
* Provide person-led, strengths-based, trauma-informed, and recovery-oriented psychosocial support.
* Co-design goal plans with participants to improve outcomes through psychoeducation and capacity building.
* Engage in comprehensive risk assessment to ensure participant and staff safety and inform appropriate support planning.
* Recognising and responding to suicide risk, self-harm and other vulnerabilities including co-developing safety planning, referrals to appropriate clinical support service.
* Demonstrated experience in managing challenging behaviours and emotional distress in a calm, safe, and respectful manner.
* Coordinate referrals, advocate for service access, and support transitions between services.
* Facilitate the MyndKind model to ensure quality, continuity, and coordination of care.
* Conduct regular reviews of participants' support needs.
* Build strong partnerships through service mapping, networking, and collaboration in the Mackay region.
* Deliver support flexibly—office-based, community outreach, or telehealth—with a preference for face-to-face engagement.
* Communicate effectively with internal teams, the Journey Coordinator Lead, the NQPHN MyndKind Team, and external stakeholders.
* Accurately record interactions in RediCASE, ensuring information is current, relevant, and accessible to appropriate MyndKind staff.
* Maintain strict confidentiality and adhere to privacy protocols for all sensitive information.
* Engage with people experiencing mental health distress, their carers, families, and community supports.
* Collaborate with the North Queensland MyndKind team, including NQPHN, Journey Coordinator Lead, and other Journey Coordinator teams.
* Collaborate with diverse organisations such as not-for-profits and government agencies.
* Represent NQPHN MyndKind and Footprints Community on relevant committees, advisory groups, and events as they relate to the role and organisational objectives.
* Perform other duties as assigned by the Direct Supervisor.
General Responsibilities
* Ensure regular attendance at team and staff meetings, including participation in quality practice activities and training.
* Participate in quality improvement activities, audits, and certification.
* Support the collection, maintenance, and reporting of program data, including key performance indicators, outcome measures, and organisational reporting requirements, as directed by the Mental Health Team Leader.
* Actively engage in supervision mechanisms.
* Contribute to any other activities that support the operation of the MyndKind model as required.
* Complete other reasonable duties as required to meet the objectives of MyndKind in the North Queensland Primary Health Network region.
* Follow all organisational policies and procedures
* Participate in monthly supervision sessions and ongoing learning and development opportunities.
* Promote and maintain a safe and healthy workplace, ensuring adherence to WHS policies and procedures.

KEY SELECTION CRITERIA
Essential
* Proven experience in building partnerships and collaborations that improve participants outcomes and service delivery.
* Strong ability to develop and maintain genuine, respectful relationships with individuals accessing MyndKind.
* Demonstrated capacity to build rapport and trust through a recovery-focused, person-centred, strengths-based, and trauma-informed practice framework.
* Sound understanding of mental health challenges and their impact on individuals’ experiences and support needs.
* Demonstrate professional and ethical practice.
* Advanced communication, interpersonal, and organisational skills.
* Understanding of mental health challenges and their impact on individuals.
* Knowledge of, or the ability to quickly acquire knowledge of, regional supports and services to enhance service navigation.
* Ability to work both collaboratively and independently.
* Have the ability to meet deadlines in a high demand work environment and to exercise judgement and initiative.
* Proficient in the use of Microsoft Office Suite and digital client management software.
* Holds a current Queensland C-Class Driver’s Licence.
* A Federal Police Check, Blue Card, and National Worker Screening or Yellow Card are required for this role.
DESIRABLE
* Current First Aid Certificate and CPR
* Certificate IV in Mental Health or Diploma in Mental Health
* Relevant qualifications or knowledge of theory/ frameworks
* Skills in counselling and motivational interviewing
* A lived experience of holistic recovery
