Role Overview
The role of the Care Team Leader is to lead the team of Community Care staff who provide direct care to our clients. The role ensures that staff have a good understanding of individual client needs and preferences and can provide services tailored to the needs of individual clients, delivered within budget and service expectations.
The Care Team Leader is responsible for leading, supporting, and mentoring their team, and works in partnership and consultation with the Care Manager, Clinical Care Manager, Client Liaison Manager, and Regional Area Manager, where required. Manage the recruitment of new Care Workers and ensure all working validations, education, development plans and resources are available.
Responsibilities
* Lead, support, and mentor the team of Community Care staff.
* Ensure staff understand individual client needs and preferences, delivering services tailored within budget and service expectations.
* Partner and consult with the Care Manager, Clinical Care Manager, Client Liaison Manager, and Regional Area Manager as required.
* Manage recruitment of new Care Workers and ensure all working validations, education, development plans and resources are available.
* Provide performance coaching and manage team in a multi‐site environment.
* Maintain familiarity with Home Care Packages Program, CHSP services, Aged Care Quality Standards and Commonwealth Home Support Programme.
* Hold a current driver's licence and maintain a safe driving record, being willing to travel between clients, sites and services.
* Possess strong organisational, administrative and computer skills, empathy and patience.
Qualifications
* Certificate IV in Aged Care (desirable).
* HR or similar degree.
* Minimum of 2 years relevant industry experience.
* Knowledge of Home Care Packages Program, CHSP services, Aged Care Quality Standards and Commonwealth Home Support Programme.
* Experience in performance coaching and team management in a multi‐site environment.
* Current driver's licence with a safe driving record.
