Access Support Coordination – Support Coordinator

Join our growing team in supporting participants in implementing their NDIS plans, ensuring they understand their options, and making confident, informed decisions. You'll play a key role in coordinating supports while promoting participant capacity, independence, and effective use of funding.

Key Responsibilities
* Support participants to build meaningful and connected lives beyond funded supports
* Assist participants in understanding and implementing their NDIS plans
* Coordinate supports efficiently to maximise participant outcomes and funding utilisation
* Help gather supporting evidence that reflects changing participant needs and future funding requirements
* Identify opportunities in areas including employment, recreation, education, and housing
* Advocate for participants and liaise with mainstream services to ensure responsibilities are met
About You
* Proven experience in a Support Coordination role or similar within the Disability sector
* Qualifications in Disability, Community Services, Social Work, or a related field
* Strong knowledge of the NDIS and related processes
* Ability to manage KPIs and billable hour targets effectively (highly regarded)
* Excellent interpersonal and problem-solving skills with a participant-focused approach
* Current driver licence, Working with Children Check, and NDIS Worker Screening Clearance required
