Community Care Specialist
We are seeking a skilled Community Care Specialist to join our team in Mount Isa, Queensland.

* You will be responsible for empowering unpaid carers through emotional support and tailored coaching, as well as providing Carer Gateway services within an outreach framework.
About the Role
* You will work with a diverse range of clients, set your own schedule, and have the chance to see the real difference your work makes as you enable rural communities to live and receive high-quality holistic care.
* In addition to the foundations of the Carer Support Officer role, your responsibility will be to deliver high-quality mental health assessment, treatment, and intervention services, as well as psycho-social supports to clients of the Carers Gateway.
Key Responsibilities:
1. Creating strategies to identify carers and communities and support them to access the Care Gateway services and support.
2. Support Carers with a high level of customer service ensuring Carers are listened to with patience and non-judgement.
3. Conduct registration and complete STAR services in order to commence service delivery.

Please note:
* You must have the right to live and work in Australia.
* You will be required to travel by road and/or in small planes to communities distant from your catchment area on a regular basis, so must be comfortable with this aspect of the role.
* A 'C' class Driver's License (manual preferred) is also an essential requirement.
