Community Care Coordinator
About the Role:
We are seeking a dedicated and organized Community Care Coordinator to join our team in Karratha, Western Australia. The successful candidate will be responsible for coordinating community care services to ensure the wellbeing of individuals with disabilities.
Main Responsibilities:
1. Schedule Appointments and Transportation: Coordinate community care services to meet client needs, ensuring timely and efficient delivery of support.
2. Conduct Comprehensive Assessments: Determine clients' requirements and implement NDIS care plans tailored to their individual needs, promoting their overall wellbeing and safety.
3. Maintain Quality Standards: Monitor the delivery of care services, guaranteeing that clients receive the necessary support and meet quality expectations.
4. Collaborate with Healthcare Professionals: Work with healthcare professionals to provide comprehensive support for clients, ensuring seamless integration of care services.
5. Establish Relationships: Develop and maintain effective relationships with stakeholders to ensure the best possible outcomes for individuals with disabilities.
6. Record-Keeping: Maintain accurate records of client information and service delivery, utilizing data to inform future service improvements.
Requirements:
* A degree or diploma in a relevant field such as Community Services, Social Work, Nursing, or a related discipline is required.
* Prior experience in community care or a similar role is highly valued but not essential.
