Overall purpose
of the program:
The My health for life team is
part of the Navigation and Access team, and the Stroke Programs, Research and
Innovation division. The My health for life Workplace Project Coordinator
supports the delivery of My health for life, a collaborative program led by
Diabetes Australia, in partnership with Stroke Foundation, the Ethnic
Communities Council of Queensland and the Queensland Aboriginal and Islander
Health Council.
My health for life is a free
healthy lifestyle initiative supporting adult Queenslanders to make healthier
lifestyle choices and reduce their risk of developing chronic conditions such
as stroke, type 2 diabetes and heart disease. My health for life is funded by
the Queensland Government and led by Health and Wellbeing Queensland
Stroke Foundation's role in My
health for life is to work closely with priority communities to improve access
and engagement with the initiative. Priority target cohorts include:
* Men
* People living
in rural and remote areas (MMM 2-7)
* People living
in low socioeconomic areas (SEIFA Quintile 1)
* Workers in
workplaces
Overview of the
position
Reporting to the My health for
life Manager, the My health for life Workplace Coordinator is responsible for
coordinating and delivering strategic workplace engagement projects across
Queensland. This role works to increase program reach and participation amongst
priority populations to support Stroke Foundation program deliverables. This
role also identifies, recommends and participates in continuous quality
improvement to enhance activity outcomes.
Key
responsibilities:
* Apply
preventive health knowledge, principles and practices to identify,
develop, deliver and evaluate the delivery of large-scale engagement
projects in collaboration with partner organisations across Queensland
* Identify,
engage and manage collaborative stakeholder relationships across sectors
to build health promotion opportunities
* Identify,
report and actively manage risks associated with activity delivery and
stakeholder engagement
* Coordinate the
implementation of My health for life activities, working closely with My
health for life provider organisations who deliver the program in their
communities
* Develop
university student-led projects and provide supervision and mentoring to
students during their placements
* Deliver My
health for life health check and promotion activities in community and
workplace settings
* Contribute to
service planning, organising, monitoring, reporting and quality
improvement as required
* Travel across
Queensland as required
Skills,
Knowledge and Experience
* Tertiary
qualifications in Health, Health Promotion, Public Health or related field
(essential)
* Post graduate
qualification in Management, Public Health, Health Promotion or related
field (desirable).
* Demonstrated
ability to support the development, implementation, and evaluation of
evidence-based health promotion projects
* Excellent
interpersonal and communication skills, including relationship management,
negotiation and influencing
* Strong written
communication skills including community promotion, report and submission
writing
* Strategic
thinking skills and the ability to apply insights to planning, problem
solving, and continuous improvement
* Knowledge of
Chronic disease prevention, health literacy, and community engagement
* Planning,
managing and evaluating large-scale health promotion interventions
* Stakeholder
engagement and relationship management
* Delivering
public health programs in community settings
* Working with
priority populations, including people from culturally and linguistically
diverse backgrounds and Aboriginal and Torres Strait Islander communities
(desirable)
