Homelessness Case Manager
We are seeking a dedicated professional to provide case management support to individuals who are homeless or at risk of homelessness.
The successful applicant will develop and action case management plans, providing practical and personal support to help individuals achieve independent living.
* Developing and reviewing case management plans to meet individual needs.
* Providing crisis budgeting, information, advice and community referrals.
* Conducting assessments of participant needs, including risk assessments and safety planning as needed.
* Advocating and assisting participants to access housing and specialist services.
Mandatory qualifications include:
* Certificate IV in Community Services or similar.
