This isn't your typical case management role.
You'll sit at the intersection of service delivery, relationship management, and growth, owning service delivery while actively expanding our services across Sydney and NSW.
It's a hands‐on, strategic role where you'll have real autonomy, influence outcomes, and directly contribute to both client success and business growth.
What You'll Be Doing
* Build and manage strong relationships with participants, families, and key stakeholders.
* Drive new referrals through your networks and industry connections.
* Manage end-to-end onboarding, ensuring a smooth and compliant experience.
* Oversee ongoing service delivery, ensuring quality, consistency, and outcomes.
* Coordinate NDIS supports including In‐Home Supports, Supported Independent Living (SIL), Community Access & Participation, Short Term Respite, and more.
* Develop and maintain care plans, service agreements, and support schedules.
* Monitor client progress and continuously improve service delivery.

What You'll Bring
* Strong, established networks across the NDIS sector.
* Proven experience in NDIS service delivery and case management.
* Solid understanding of NDIS funding, planning, and compliance.
* Excellent communication and negotiation skills.
* High level of organisation and attention to detail.
* Ability to manage multiple priorities in a fast‐paced environment.
* NDIS Worker Screening Check.
* Valid driver's licence.
