Overview
We are seeking a Care Manager to join a residential aged care service and oversee the delivery of high-quality clinical care. This role will play a key part in ensuring the home maintains strong clinical governance and compliance with the Aged Care Quality Standards, particularly as the service enters an accreditation window. Working closely with the leadership team, the Care Manager will support clinical operations, guide the nursing and care teams, and ensure documentation and care plans meet the highest regulatory and professional standards. This role would suit an experienced Care Manager, or a strong Clinical Care Coordinator looking to step into their first Care Manager position, provided they have excellent written communication skills and a strong understanding of clinical documentation and care planning.
Key Responsibilities
* Oversee clinical care delivery and ensure compliance with the Aged Care Quality Standards.
* Support and mentor the clinical and care teams to deliver safe, person-centred care.
* Ensure accurate and high-quality clinical documentation and care planning.
* Assist in preparing the service for accreditation and regulatory reviews.
* Review incidents, risks, and clinical indicators to support continuous improvement.
* Work collaboratively with the leadership team to maintain strong clinical governance frameworks.
* Build positive relationships with residents, families, and internal stakeholders.
Key Requirements
* Registered Nurse (RN) – essential.
* Previous experience as a Care Manager or Clinical Care Coordinator within residential aged care.
* Strong understanding of Aged Care Quality Standards and accreditation processes.
* Excellent written communication skills, particularly in relation to clinical documentation and reporting.
* Ability to lead and support multidisciplinary care teams.
* A proactive approach to quality improvement and clinical governance.
