About the Role
As the Home Care Partner, you'll be responsible for a caseload of up to 50 clients, ensuring top-tier service delivery, and supporting your care team to deliver person-centred care that promotes independence and quality of life.
Key Responsibilities:
* Leading and mentoring a team of care workers
* Developing and reviewing client-centred care plans
* Ensuring compliance with aged care standards and regulations
* Conducting regular client assessments and service reviews
* Liaising with healthcare professionals, clients, and families
* Managing care budgets and service agreements
* Promoting continuous improvement and quality assurance
What We're Looking For
* Registered Nurse (RN) or Enrolled Nurse (EN) with current registration
* Excellent interpersonal and communication abilities
* Strong knowledge of aged care regulations and home care best practice
* Minimum 1-2 years of experience in aged care or home care is required
* Proficiency in using electronic health records or care management systems
* Australian work rights are required for this position
