About the Role As a Restorative Care Partner, you'll be a trusted advocate, guide, and connector in each participant's wellbeing journey. 
You'll play a key role in driving proactive, person-centred support by: Engaging with participants, families, and community stakeholders Conducting in-depth assessments and coordinating personalised restorative care plans Providing ongoing advice, guidance, and care navigation Ensuring quality care delivery, risk management, and continuous improvement In this role, you will: Conduct home visits to understand each person's unique needs and aspirations Develop tailored care plans that empower individuals to take control of their health and lifestyle Lead and collaborate with a multidisciplinary team to deliver seamless, integrated services Build genuine, trust-based relationships with clients and their families Champion quality, innovation, and excellence in care delivery About You You're someone who wants to make a real, lasting difference. 
You bring compassion, leadership, and a strong understanding of restorative care. 
You have: Tertiary qualifications in Allied Health, Nursing, or a related field is essential Experience in restorative care, allied health, or community services Experience in care coordination, restorative or palliative care (highly desirable) Knowledge of Aged Care Quality Standards, Support at Home program, and rights-based care A proactive, person-first approach and the ability to connect with empathy Solid communication and leadership skills within team-based environments Confidence to work independently while contributing to a supportive, collaborative team A commitment to learning, growth, and best-practice care In-depth knowledge of reablement, functional rehabilitation, and person-centred care Strong IT skills, including proficiency in Microsoft Word, Excel, and client management systems Budget Reporting Experience, or willingness to learn You're also proficient in: Risk identification and management Quality assurance and compliance Assessment tools and outcome-based care planning Applications close: Sunday, 28th September at 11:59pm Why Join ADSSI?
