We have an exciting opportunity for an experienced social worker to join our client's Community Mental Health Team.
What will your responsibilities be?
You will contribute to the operational delivery of an effective Adult Social Care Service as part of an integrated Community Mental Health Team.
Your responsibilities will include safeguarding, support planning, and promoting the wellbeing of vulnerable adults and older people, continuously improving outcomes.
You will identify individual needs through assessment, plan and co-design individual support plans, and ensure regular monitoring and reviews of care plans are carried out.
You will be responsible for social work provision, including managing your own caseload while maintaining the reputation of the Department and Council and complying with national and service procedures and guidance.
We are looking for a social worker experienced in the application of the Mental Health Act, Mental Capacity Act, and Care Act.
You should have experience in hospital discharge from a mental health ward, attending tribunals, completing social circumstance reports, and assessing and sourcing support based on individual needs.

Qualifications and Experiences
To be successful in this role, you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
