We are proud to partner with Ramsay Connect on the Diabetes Using Our Strengths Service (DUOSS) pilot research program. This innovative service combines remote patient monitoring, optimized diabetes medicine management, education, and group support to help Aboriginal and Torres Strait Islander Elders manage their type 2 diabetes.
This role will lead the delivery of culturally safe and appropriate diabetes care for First Nations peoples enrolled in DUOSS in Bundjalung County. Working within a multidisciplinary team, you will provide clinical care to improve patient health outcomes. Utilizing strong interpersonal and collaborative skills, you will work in partnership with local healthcare providers and organizations to improve health outcomes.
This part-time fixed-term position requires some travel to Bundjalung Country. The project is funded by the Australian Government's Targeted Translation Research Accelerator program.
About Your Responsibilities: You will deliver majority of care virtually. Key responsibilities include:
* Titrating and/or optimizing diabetes medicines with authority from the Elders' GP
* Providing tailored education to Elders based on their clinical and cultural needs, informed by Remote Patient Monitoring
* Care Coordination
* Management of patient monitoring data
* Providing advice and input for clinical escalations as a result of remote patient monitoring results and/or questionnaire results
Your Expertise and Qualifications:
* AHPRA registration with relevant qualification as a credentialed Diabetes Educator with currency of practice
* Knowledge of the health issues affecting Aboriginal and Torres Strait Islander people with a strong commitment to improving health outcomes
* Strong interpersonal skills with experience working in a multidisciplinary team to deliver outcomes for complex patients
Desirable Skills:
* Previous experience working with Aboriginal and Torres Strait Islander people and/or communities
* Experience in working in virtual care
* Living on Bundjalung Country
