Case Manager for Family Violence
We seek a dedicated and compassionate Case Manager to provide support to women and children experiencing family violence.
About the Role
This role involves delivering case management services in a trauma-informed approach, spanning intake, assessment, crisis response, and ongoing support.
The ideal candidate will be able to build respectful relationships with clients, empower them to self-determine their goals, and facilitate access to wrap-around services and advocacy.
Responsibilities
* Deliver high-quality case management aligned with established practice standards and best practices.
* Complete comprehensive assessments identifying client needs, risks, and strengths from intake to exit.
* Develop individualized case plans in partnership with clients addressing unique goals and circumstances.
* Regularly monitor progress adapting plans to ensure continued relevance and effectiveness.
Requirements
* A degree in Social Work, Community Development, or related field is essential.
* Experience in family violence and/or child wellbeing work is highly desirable.
* Strong case management skills and ability to work independently are required.
* Excellent time management and organisational capabilities are necessary.
* Commitment to feminist practice empowering vulnerable populations is essential.
* A valid Working with Children Check is mandatory.
