Prince of Wales Hospital - Assistant in Nursing, Transitional Aged Care Program (TACP)

Join our team of energetic and passionate health professionals Transitional Aged Care Program (TACP), is a multi discipline, home based therapy and nursing program available to older inpatients upon discharge from their acute hospital admission.

This Assistant in Nursing (AIN) role is in the Community setting. We are based on the Prince of Wales Hospital Campus, Randwick. The AIN will be part of a multi discipline team that includes Physiotherapists, Occupational Therapists, Social Workers, Community Aides, a Clinical Nurse Consultant and Registered Nurses. You will visit clients who have been discharged from hospital and are with Transitional Aged Care Program (TACP) for a period up to 12 weeks.

Transitional Aged Care Program (TACP) is a multi discipline home based allied health and nursing rehabilitation program for older people discharged from hospital.

You will travel in the LGA of Randwick - Botany area. We use Hospital cars / fleet vehicles. A drivers Licence is required and all vaccinations as per NSW Health policy.

Where you'll be working

This role is in the Community setting, we are based on the Prince of Wales Hospital Campus, Randwick. The Assistant in Nursing is a new position and is part of a multi discipline team called Transitional Aged Care Program (TACP). This team includes Physiotherapists, Occupational Therapists, Social Workers, Dietitian, Registered Nurses and an Administration Assistant.

We use Hospital fleet vehicles. and travel in the LGA of Randwick - Botany area. A current NSW Drivers Licence is required and all vaccinations as per NSW Health policy.

TACP is post-acute intervention for up to 12 weeks. It is rehabilitation in the home for older people (Over 65yrs and 50 years of age for Aboriginal people) who live in the Randwick – Botany LGA.

There are 32 clients on TACP at any one time. We see the person in their home on the same day as discharge from hospital. All clinical handovers are provided from the hospital Nursing and Allied Health teams.

What You'll Be Doing

You will be working in a multi discipline team. You will be providing assistant in nursing care - such as assisting clients personal care, medication management and monitoring, accessing the community and medical appointments, transporting clients, monitoring daily activities, and empowering the clients towards independence.

We advocate for shared decision-making, collaborative relationships, compassionate and person-centred leadership, and supporting innovative practices.

The nursing vision for Prince of Wales Hospital is 'Transforming Person-Centred Care Together'. This approach to all nursing practices ensures we collaborate together on all aspects of healthcare, inclusive of staff, placing the person at the centre of shared decision making underpinned by respect, mutual understanding and facilitated choice.

Our staff, through facilitation, are empowered and engaged in the shared decision making process in order to influence practice. This is achieved by working together in an environment which supports person centredness through evidence based care, learning and enquiry.

The Transitional Aged Care Program based at Prince of Wales Community Health provides time limited, multidisciplinary support and low level therapy to clients living in the community.

The Assistant in Nursing (AIN) works in the Transitional Aged Care Program (TACP) which provides a package of services for up to 12 weeks for clients over the age of 65 years who after admission to hospital, meet a set criteria for a home-based aged care package and who may have the potential to manage at home with rehabilitation and support services.
* This role engages directly in the provision of care, which assists the TACP clients with personal and domestic activities of daily living, function and mobility tasks, community access and client transport and be supervised by the Registered Nurse (RN) and all members of the multi-disciplinary TACP team.
* The AIN's role also works within a multidisciplinary team to provide therapeutic intervention and support to aged clients delivered in their homes following a period of sub-acute hospital care. The purpose of this intervention is to improve their physical function, avoid further hospital presentations and to maintain daily living skills, in accordance with the restorative goal ethos. Intervention should improve the client's capacity to maintain or regain independent living, reducing the burden of care for carers and delaying the need for residential care.
* Provision of support to the nursing team in the delivery of nursing care in the community care environment as directed by the Registered Nurse
* Provide direct care activities to patients in accordance with the nursing care plan and under the supervision of a Registered Nurse.
* Assist Registered and Enrolled Nurses with patient care interventions as directed.
* Contribute to collecting accurate health care information and maintaining accurate health care documentation as required.
* Communicate effectively with patients and other health care team members in accordance with appropriate protocol and within the scope of Transitional Aged Care Program (TACP).

Selection Criteria
* Certificate III in Health Services Assistance (Descriptor: Assisting in Nursing Work in Acute Care) or equivalent and Current unrestricted NSW Drivers licence, Class C
