You will deliver holistic care that aligns with Consumer Directed Care principles, and liaise with relevant stakeholders to ensure effective care delivery.**Responsibilities include:*** Promotes and delivers holistic care focusing on client needs and rights through assessment, prevention, early detection, referral, and health promotion activities.* Delivers nursing care in adherence with relevant legislation and organisational policies* Provides delegated nursing care and individualised case management for residents under the supervision of a Registered Nurse.**About You****To be successful in this role, our preferred candidate will have:*** Diploma of Nursing (required)* Experience in aged care (highly desirable)* Strong knowledge of Quality Care Service Management, Clinical Practices and Standards, Legal & Regulatory Compliance and Patient Case Management skills.* Excellent communication, adaptive thinking, planning and prioritisation, and cross-functional team working skills.* Prior to commencement - complete a National Police Check and or a NDIS Workers Check (or willingness to obtain)**About this location:**Raffles, a 95 bed residential aged care facility located near the beaches of the Southern Gold Coast.
