Job Description:
A Clinical Care Manager is responsible for overseeing daily operations of a care home, ensuring the delivery of high-quality, person-centred care through advanced clinical practice, staff supervision, and compliance with aged care standards.
The role involves providing clinical guidance and support to nursing and care staff, ensuring care is delivered in line with residents' goals, preferences, and assessed needs. This includes overseeing care planning, assessments, case conferences, and AN-ACC documentation, as well as ensuring compliance with relevant aged care legislation, regulations, and best practices.
The successful candidate will have strong leadership skills, ability to lead, mentor, and support a multi-disciplinary team, excellent written and verbal communication skills, and well-developed skills in time management, prioritisation, and decision-making.
Required Skills and Qualifications:
* Current AHPRA registration as a Registered Nurse
* Minimum 3 years' post-registration experience in aged care
* Strong understanding of clinical governance and aged care standards
* Proven ability to lead, mentor, and support a multi-disciplinary team
* Excellent written and verbal communication skills
* Well-developed skills in time management, prioritisation, and decision-making
* Computer literacy and proficiency in clinical documentation systems
* Must hold, or be eligible to obtain, an NDIS Worker Screening Check (or a current AFP check with an active NDIS application)
* Full working rights in Australia
* Up-to-date influenza vaccination and willingness to comply with applicable health directives
