About the role
Clinical Care Liaison – a proactive, compassionate Registered Nurse or Allied Health Professional with excellent communication skills and strong attention to detail, working with our Statewide Progressive Neurological Disease Service (SPNDS).
What your day might look like
* Attend neurology, psychiatry, and/or palliative care clinic appointments, via telehealth or in clinic face‐to‐face, to ensure continuity of care.
* Arrange follow‐up referrals, appointments, care plans and management strategies arising from clinic appointments.
* Telephone triage and assessment of needs with patients, families, and care providers.
* Undertake inter‐disciplinary appointments with patients to provide holistic assessment & management.
* Provide specialist advice and support to patients, family members, and care providers over the telephone, via telehealth or in‐person clinic appointments.
* Participate in multi‐disciplinary team meetings both internally and with external support providers.
* Conduct research activities.
What you bring
* Motivation, energy, enthusiasm and the right attitude – even if you do not have all desirable experience.
* AHPRA registration as a Registered Nurse by the Nursing and Midwifery Board of Australia or Allied Health Professional equivalent based on clinician specialty.
* Experience in one or more of: Progressive Neurological Diseases, Palliative Care, Mental Health, Disability, NDIS or Community Health.
* Experience in care planning and care coordination.
* Proven ability in managing patients with complex psychosocial, physical and/or mental health needs.
* Ability to work independently and autonomously in complex situations within scope of practice.
* Understanding, respect and application of principles of dignity of risk to maximise quality of life.
* Ability to form therapeutic relationships with patients, families and carers enabling provision of psychosocial and emotional support.
* A desire to engage in research, education and ongoing quality improvement activities.
