**What you'll be doing in this role**
We require qualified aged care workers to care for clients who are transitioning out of the hospital and into the **_Northern Sydney Local Health District's_** **Transitional Aged Care Program**.
In this role, you will empower independence and improve the quality of life for clients living in the **Northern Sydney Area **as you provide practical support and companionship for older people returning home after a stay in the hospital. You'll be required to travel to client's homes and communities using your own car.
**Key Responsibilities**
- **Help at Home**: Housekeeping, grocery shopping, and meal preparation.
- **Personal Care**: Assisting with personal hygiene, showering, dressing, mealtime, and medication administration.
- **Transport**: Driving clients to appointments or on shopping trips.
- **Social and Wellbeing**: Accompanying clients on outings and social support activities.
**Qualifications and Experience**
- Certificate III in Aged Care, Individual Support or equivalent (preferred).
- Experience in providing client-centered care.
- Current First Aid certificate.
- Valid driver's license and access to a comprehensively insured vehicle.
