Key Responsibilities:

* Collaborate with clients to develop and implement personalized support plans
* Facilitate access to community resources and services, promoting social inclusion and participation
Requirements:
* Certificate III in Disability Services or equivalent qualification
* Valid driver's licence for transportation of clients (if required)
* Proven experience working with people with disabilities, including those with complex needs
