- Metro Melbourne, West, North, Inner East regions of Melbourne
- Part-time (weekday day shifts and afternoons with every 2nd weekend)
- Minimum part time engagement preferred 3-5 days per week- working hours with 7.00am-9.00pm.

We’re looking for experienced Community Team Leader to work in our In Home Nursing team in the Metro Melbourne, West, North and Inner East. Our In Home Nursing Service provides in-home and post-acute care for clients, HCP, NDIS and HITH services. Working in different homes every day, your ability to quickly analyse your client’s needs and work with them in a respectful way is vital. This is not an office based role and requires travel with you visiting homes in the areas of the Central metro, West, North of metro Melbourne.
A typical day will have you:
- Undertaking nursing duties for the client to maximise the client’s wellbeing and healthcare at home
- Monitoring and advising of risks in and around the client's environment
- Providing baseline assessment information, ongoing care recommendations and/or appropriate interventions
- Ensuring the continuity of client care by liaising with hospitals, other community services, doctors and allied health professionals
- Co-ordinating a written nursing care plan for clients and assisting with the implementation of this plan
- Observing and promptly reporting any changes in a client’s behaviour or health/wellbeing
- Educating junior staff
We’re looking for someone with:
- A Registered Nurse (Division 1) with current AHPRA registration
- 3 + years experience in Hospital in the Home, Emergency, Critical Care, Acute Care and/or Community Nursing.
- Exceptional wound care experience
- IDC/SPC experience preferred
- Self-driven with excellent client/patient management and clinical skills
- Customer service focused, with the ability to provide client and family education and support
- Friendly, positive, and able to build relationships that make a difference
· Excellent communication (both oral and written), engagement & presentation skills.
· Passionate, driven, positive and adaptable.
· Able to work autonomously and within a team.
· Fully licensed to drive in Victoria with a reliable fully insured vehicle
