Clinical Coordinator Opportunity – Deliver Excellence in Country Health Connect

We are seeking a dedicated Clinical Coordinator to deliver high-quality, person-centred nursing care across home care and community health settings through the Country Health Connect program. Reporting to the Nurse Unit Manager, you will play a pivotal role in supporting clients, providing professional nursing services, and contributing to the development and supervision of less experienced staff.

The Clinical Coordinator (RN/M2C)applies clinical experience, specialist knowledge, and professional competence to plan, implement, and evaluate a comprehensive and integrated range of services tailored to the needs of Country Health Connect clients. This role provides leadership, supervises and guides less experienced staff, and manages assigned portfolios in line with career development plans. Employees at this level accept accountability not only for their own nursing practice but also for delegated activities and team outcomes.

The Registered Nurse (RN/M1)provides nursing services to clients in home care or community health settings, consolidating knowledge and skills from novice to proficient practitioner levels. Working under the direction of the Clinical Coordinator, the RN/M1 applies professional standards and clinical knowledge to deliver safe and effective care. Employees at this level accept accountability for their own practice standards and for activities delegated by senior staff.

What We're Looking For:

RN/M2C

* Registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
* Registered Nurse/Midwife with at least 3 years, full time equivalent, post registration experience.
* Experience in the use of computer packages e.g., Microsoft Word, Excel.
* Demonstrated competence in the relevant area of nursing practice in accordance with the appropriate standards of practice.
* Strong clinical background in care of the older person.
* Demonstrated ability to competently undertake holistic client medical assessments, appropriate memory and wellbeing assessments.
* Experience in developing clinical policies and procedures.
* Experience in the leadership and direction of student nurses, enrolled nurses, and less experienced registered nurses.

RN/M1

* Registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate. AND
* Must be endorsed and credentialled in order to prescribe medications or to order diagnostics to the scope and context of their practice.
* Minimum of 2 years post graduate experience.
* Demonstrated competence in the relevant area of nursing practice in accordance with the appropriate standards of practice.
* Experience in the use of computer packages e.g., Microsoft Word, Excel.

Applicant Information

This is a part time vacancy, working 45.6 hours per fortnight and salary will be adjusted according to actual hours worked.
