Your responsibilities will include conducting thorough assessments, developing comprehensive care plans, and providing ongoing support to ensure optimal outcomes.
Key Responsibilities:
* Assessment Skills: Conduct thorough assessments of individuals' needs, identifying their physical, emotional, and social requirements to determine appropriate care.
* Care Planning: Develop tailored care plans that consider factors such as medical treatments, personal assistance, emotional support, and social activities to ensure holistic care.
* Consumer-Focused: Prioritize consumers' choices, preferences, and goals, actively involving them in decision-making and empowering them to make informed choices about their care.
* Liaison with Health Professionals: Communicate and collaborate with health professionals to ensure effective implementation of care plans and necessary adjustments or referrals are made.
* Visiting Consumer: Visit consumers in their homes to assess their well-being, monitor progress, and provide ongoing support, building rapport and addressing concerns.
* Empathy and Communication: Possess strong empathy and communication skills, listening actively, understanding needs and emotions, and communicating effectively to build trust and establish positive relationships.
* Problem-Solving and Advocacy: Be skilled at problem-solving, identifying challenges, and finding solutions, advocating for consumers' rights and navigating obstacles or barriers.
* Organisation and Time Management: Handle multiple cases simultaneously, requiring excellent organisational skills to manage workload, prioritise tasks, and meet deadlines effectively.
* Compassion and Patience: Approach individuals with compassion and patience, providing reassurance, encouragement, and emotional support when needed.
Requirements:
* Essential Qualifications: Degree in Social Work, Nursing, or related field.
* Desirable Qualifications: Relevant certifications, experience working with vulnerable populations.
* Skills: Excellent communication and interpersonal skills, ability to work independently and as part of a team.
