Position Overview

As a Support Coordinator / Community Worker at Heed WA, you will play a vital role in helping our clients live fulfilling lives in the community.


Main Duties

* Work directly with NDIS participants to support them in achieving the goals outlined in their individual support plans.
* Provide practical day-to-day assistance including personal care, household tasks, life skills coaching and routine support.
* Encourage client independence by helping build capability in areas such as communication, mobility, social interaction and community engagement.
* Support clients in attending appointments, social outings, shopping and community activities, ensuring they feel confident, safe and included.
* Develop positive and trusting working relationships with clients and families that reflect Heed WA's respectful, client-first philosophy.
* Observe changes in clients' needs and communicate these to supervisors and support coordinators to ensure timely plan adjustments.
* Maintain accurate documentation, progress notes, incident reporting and service records in line with NDIS compliance requirements.
* Work collaboratively with behaviour support specialists, recovery coaches and the broader care team to deliver integrated care.
* Uphold Heed WA's commitment to choice, dignity and respect in every interaction.
* Represent Heed WA professionally in the community, helping build a supportive, inclusive and high-quality service experience for all participants.


Essential Requirements

* Diploma or Bachelor's degree in Community Services or a closely related field.
* Minimum 4 years of experience working as a Community Worker or in a similar support role.
* Strong communication skills and the ability to support clients with empathy and professionalism.
* Ability to work independently while maintaining a high standard of service.
* Willingness to provide hands‑on assistance with daily living tasks when required.
* Current Driver's Licence and access to a reliable vehicle
* NDIS Worker Screening Check and First Aid Certificate, or willingness to obtain them
* Understanding of NDIS practice standards and person‑centred care
