Job Overview
The Transition and Post Care Support Officer role is designed to provide direct support to young people with complex needs, aged 15-21, who are at risk of homelessness. The successful candidate will work collaboratively with Child Safety staff, government and non-government agencies to improve the quality of life for these young people.
Key Responsibilities:
* Provide specialist transition from care support that identifies and responds to the complex needs of high-risk young people.
* Promote and facilitate involvement of the young people in goal setting and planning for their transition.
* Organise the provision of specialist services that address high risk and complex needs of young people.
* Consult and develop relationships with government and non-government agencies to ensure efficient service delivery.
* Collaborate with regional line manager and senior program officer to ensure continuous quality improvement.
The ideal candidate will possess a deep understanding of the principles of the Strengthening Families Protecting Children Framework for Practice and the core elements of the Aboriginal and Torres Strait Islander Child Placement Principle.
