Role Description
The Care Coordinator works collaboratively with clients, general practitioners, clinic staff and other health service providers, to provide care services for Aboriginal people with chronic conditions to optimise health outcomes Specifically, the role supports Aboriginal people with chronic health conditions living in Congress Remote Health Services The position supports clients who are required to travel to Alice Springs or interstate for specialist appointments. Some remote travel is inherent in the position.
To be considered for this position, you will require:
* Registered or eligible to register with AHPRA as a registered nurse or Aboriginal Health Practitioner with at least 2 years clinical experience in chronic disease management and/or primary health care
* Excellent understanding of primary health care and sound clinical knowledge of chronic disease management
* Understanding and commitment to Aboriginal health, and the philosophy and practice of Aboriginal Community Control and knowledge of current issues affecting the lives of Aboriginal people.
