The Clinical Care Partner is a rewarding part-time (0.8 FTE) opportunity based in our South Morang office, supporting clients within the aged care sector through the delivery of high-quality, person-centred care. This role is ideal for a compassionate and experienced professional who is committed to improving outcomes for older people from diverse cultural backgrounds.
The position will play a key role in supporting service delivery through a period of transformation, ensuring alignment with the new Support at Home Program (SAHP) framework and funding model. Working collaboratively within a dedicated team, you will contribute to service excellence, enhance the client experience, and support the continuous improvement and growth of our aged care services. Key Selection Criteria
* You must be a Registered Nurse with demonstrated Case Management experience in the aged care sector and experience delivering in-home aged care services under Home Care Packages, and/or the Support at Home Program. Please clearly outline this experience in your cover letter.
* Have extensive understanding of the challenges, needs, and issues affecting older people, their families, and carers.
* Have demonstrated knowledge and understanding of the Support at Home Program, and support systems within the aged care sector, including funding mechanisms and associated requirements.
* Have proven experience in delivering person-centred care and support with the ability to work within service models that promote client empowerment.
* Capacity to work with a diverse range of people, including people with a disability, diverse communities, allied health and other service providers, assessment agencies and referrers.
* Excellent interpersonal skills and ability to work in a multidisciplinary team.
* Ability to genuinely embody and promote our values.
