The Role
Make a meaningful impact every day by supporting people living with dementia as they transition from hospital to home or into residential care.
As a Dementia Support Coach, you’ll provide practical, relationship-based support, modelling and coaching for clients, carers and care staff across a variety of settings.
This position offers professional growth and the opportunity to contribute to an innovative new project.
This role is offered as a Fixed Term Contract until March 2028 and is based out of Albury.

Responsibilities
* Provide on-the-ground support and implement relationship-based strategies for clients, carers and care staff
* Model and coach best-practice approaches to help carers and aged care staff understand and support people living with dementia
* Implement recommendation plans with guidance from the Team Leader, Dementia Consultant and Senior Consultant
* Facilitate and support the trial and implementation of brokerage items for clients and carers
* Collaborate with peers to build capability across aged care teams and support quality, person-centred care
What do we need from you?
* Strong organisational and time-management skills in a busy work environment
* Ability to work independently and collaboratively
* A commitment to excellent customer service and constructive conflict resolution
* A current driver’s licence and safe driving record
* Willingness to travel intrastate and interstate for up to five days at a time
* Flexibility to work varying hours based on client and carer needs
* Qualifications in a relevant field: Allied Health Assistant, Diversional Therapy, EN, Aged Care, Cert III/IV in Individual Support or Leisure and Health (highly desirable)
