As an Aged Care Liaison Specialist, you will play a vital role in facilitating seamless transitions between hospitals and residential care facilities. Your expertise will ensure that patients receive the best possible care, while also promoting efficient coordination of services.
Key Responsibilities:
* Foster strong relationships with healthcare providers, aged care facilities, and other stakeholders to enhance continuity of care
* Develop and implement effective strategies for improving discharge coordination, ensuring timely and smooth transfers between hospital and residential care settings
* Collaborate with multidisciplinary teams to provide comprehensive patient assessments, coordinate care plans, and implement planned transfers of care
Required Skills and Qualifications:
To be successful in this role, you will require:
* A degree in nursing or a related field
* Registration with the Nursing and Midwifery Board of Australia as a Registered Nurse
* Proven experience in aged care, preferably in a liaison or coordination role
