We are seeking Support Workers to join our team at New Horizons. We offer permanent and part-time positions in supported independent living and inclusion services.
About the Role
As a Support Worker, you will provide person-centered support to our customers, empowering them to achieve their goals and improve their well-being. You will work a rotating roster, Monday to Sunday, with flexible shifts including mornings, evenings, and weekends.
2. Provide support to improve independence, assisting with living skills, transport, social activities, medication, and personal care.
3. Offer psychosocial support to explore practical and complex needs.
4. Maintain customer records using online systems.
5. BUILD professional relationships and networks with customers, families, caregivers, and key stakeholders.
6. Enhance the customer journey through innovative approaches.
