Our program brings together a multidisciplinary team of healthcare professionals who work collaboratively to deliver tailored care that promotes recovery and wellbeing. As part of this team, you will work closely with nurses, allied health staff, and support workers to provide valuable clinical services to patients and offer guidance and assistance to their families and carers.
In this role, you will be responsible for delivering high-quality Social Work services within the Transitional Aged Care Program. You will conduct home visits as part of a service that operates daily from 8:00 am to 4:30 pm, 365 days a year. Our comprehensive onsite training, including CHIME training, will equip you with the skills and knowledge needed to meet the demands of this rewarding and impactful role.
Key Responsibilities:
* Conduct home visits to support patients and their families
* Deliver high-quality Social Work services within the Transitional Aged Care Program
* Work collaboratively with a multidisciplinary team to deliver tailored care
Required Skills and Qualifications:
* Bachelor degree in Social Work or equivalent qualification
* Unrestricted NSW Drivers Licence
