Permanent Full Time – 38 hours a week, salary packaging benefits, 5 weeks annual leave, based in Bundaberg.
About the role
Churches of Christ are looking for a Care Partner within our dynamic Home Care team. The role involves working across all Support at Home pathways, including Restorative Care, End of Life, Assistive Technology and Home Modifications, contributing to the mission and strategic goals of Churches of Christ as part of the Seniors Living team.
Key responsibilities include:
* Develop individualized and culturally appropriate care plans aligned with assessments, goals, preferences, risks and AT HM needs; ensuring care plans identify wellness and reablement approaches to meeting participant goals.
* Collaborate with relevant team members to support the coordination of services under the End of Life and AT HM Pathway (within scope), ensuring alignment with participant goals and safe integration of supports.
* Provide essential direct care services to participants as required, in response to individual care needs and service demand (such as personal care, medication support and transport).
* Deliver monthly care management interactions to all participants.
Qualifications
* Certificate 3 in Home and Community or Individual Support
* One year’s experience in home care and planning, coordinating and implementing care services in the home care environment.
* HLTHPS006 – Assist clients with medication.
* HLTAAP001 – Recognise healthy body systems.
* CHCCCS011 – Meet personal support needs.
* Currently holds, or willing to obtain a First Aid and CPR qualification.
* Currently holds, or willing to obtain a current Police Check.
Other Requirements
* A current driver's licence and willingness to use your own comprehensively insured vehicle for work purposes. (Reimbursement of $0.96 per km is provided for use of own vehicle).
* Hold valid Australian Work Rights – Unfortunately we are not currently able to sponsor.
