About The Role
The stable hand health and well-being key worker aims to improve access to health and well being services for stable employees, track riders and trainers at the Cranbourne, Pakenham, and Mornington racecourses. The program is a partnership between Melbourne Racing Club Foundation and Monash Health Community.
The incumbent will promote and facilitate eight community engagements activities per year.
The incumbent will take part in managing referrals and enquires and enquires for the stable well being program as well as service promotion to the community.
The role requires timely and appropriate individual needs assessment, therapeutic treatment and care co-ordination including supporting service user referrals to identified health services for members of the racing community.
The role will also require the implementation and facilitation of therapeutic group programs as deemed appropriate to people with identified needs.
About You
You have the ability engage with service users and link to Monash Health Services which can provide support in regards to sexual and reproductive health, rehabilitation, disability, sexual assault, youth services and case management.
The ability to manage referrals and enquiries for the services programs as well as service promotion to the community and providing oversight to this process.
The incumbent will have an up-to-date knowledge of the health sector and community health as well as an understanding of community health philosophy, programs and services.
The role requires excellent administrative, IT, organisational, communication and leadership skills, the ability to work independently and a commitment to improving the way health services are delivered for patients, in particular vulnerable people
