The Team:
The Specialist Palliative Care Service provides consultative, multidisciplinary support to patients, carers, and clinicians across both community and inpatient settings in North West Tasmania. As part of a small, dedicated team, you'll contribute to delivering best-practice care that prioritises dignity, comfort, and individual choice.
The Role:
* As a Clinical Nurse Consultant, you will provide leadership within the team in supporting palliative clients in a variety of locations. Including home visits to patients and their families, assessing conditions and needs for supports and reporting back to the specialist team.
* Working collaboratively with the wider team, you will ensure timely response to requests for support and assess response to interventions.
* This role requires excellent communication and documentation skills to thrive.

You will need:
* Excellence in communication across a variety of professional and client settings
* Experience and understanding of the palliative approach to care
* Demonstrated understanding of a collaborative consultative approach to care across the acute and primary health settings working with THS and NGO's to ensure comprehensive care supporting palliative patients.

Details of appointment:
* Permanent, full time, day worker position, working up to 76 hours per fortnight, commencing from as soon as you can join us.
*notwithstanding hours to be negotiated with the successful applicant.

Salary:
$118,297 to $125,543 per annum. Our Employer 12% superannuation contribution is on top of this amount.
* Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023.
In addition, the following allowances calculated on the salaried incremental point may be available:
* Post Graduate Allowance of up to 10 % based on qualification type

Eligibility:
Successful applicants will be required to meet the essential criteria
* Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse
* Current Driver's Licence
