Through our Access Aged Care model, we deliver co‐managed, relationship‐based care to residents across Australia, working closely with dedicated GPs, nurses and specialist clinicians to strengthen health outcomes, continuity of care and clinical oversight.
Responsibilities
* Provide regular telehealth services to residents in aged care facilities.
* Review residents, complete progress notes, manage acute issues and support ongoing clinical care throughout each virtual round.
Requirements
* Qualified Geriatrician (physician).
* Commitment to a minimum of 3+ hours per week and value flexibility in working arrangements.
