**Your new company**
This Federal Government Department delivers government programs to support ex-service men and women and their families. They are currently seeking Case Manager (Psychologist, Social Worker, Registered Nurse, Mental Health Registered Nurse, Occupational Therapist) to join their Triage and Connect Team in a full-time capacity on a 12 month initial contract.
Triage & Connect is the gateway to all supports under the client support framework. The T&C team provides a single access point for advice, assessment and response to urgent, high risk or escalated client matters identified by any internal staff and key external stakeholders. This is a very short term intervention. The majority of referrals through T&C are be resolved within the T&C team. Most of the services delivered under the aegis of the client framework are telephonic, however there may be opportunities for face to face service delivery on occasion.
**Your new role**
You will be responsible for the assessment and risk management of clients referred to the team due to issues relating to mental health, psycho-social issues and suicide risk. This process involves speaking with clients, conducting risk assessments, identifying barriers to accessing help and liaising with relevant treating professionals where consent is given.
In the case of a serious threat of harm to self or to others, you will escalate according to the client support framework procedure, which may require alerting external agencies without the client's permission. This should always be done with the agreement of an Assistant Director/Director.
You will also provide short term case management consistent with the T&C model, and may provide expert advice and support on request from other team members.
In your new role you will work closely within a Team with clinical and non-clinical staff providing assistance to clients identified as being at risk and/or having complex needs. You will be responsible for:
- Providing short-term high quality, evidence based clinical services including intake, assessment and referral, complex case management, and case coordination to a caseload of clients with identified complex and/or multiple needs.
- Maintain clinical records according to standards, collating and analysing data from client records, and preparing reports as required.
- Liaise and form partnerships internally as well as other government departments and organisations.
- Undertake calls to clients where required to offer assistance or obtain further information, where this contact is best managed by a mental health professional.
- Link clients to appropriate mental health services and occasionally provide short-term case management intervention to link clients with necessary services for ongoing support.
**What you'll need to succeed**
To be successful in this role you will possess the following:
- You're a qualified Psychologist, Mental Health Accredited Social Worker, Registered Nurse, Mental Health Nurse or Occupational Therapist registered with AHPRA or AASW.
- Experience in coordinating and assisting in the management of a caseload of complex and potentially at-risk clients in community mental health teams.
- You're an Australian Citizen.
