Position Summary :
All Support Workers have the responsibility to ensure participants receive a comprehensive range of support services. This includes traveling between participant's homes and assisting with their daily living and lifestyle requirements by providing the coordination and management while ensuring compliance with their service plan and agreements.
Key Requirements :
Current knowledge of the Disability Sector;
Behaviour management training an asset;
Previous experience supporting clients with daily routines such as : personal care, meal preparation, transport, and community engagement is desirable;
Previous experience working autonomously;
Ability to help set goals and plans to reach them
Strong written and verbal skills, particularly in recording clear and accurate participant records
Experience in the delivery of excellent customer service;
Ability to establish and maintain effective relationships;
Proven ability to work in a team and solo environment;
Demonstrated experience in managing complex workloads;
Ability to be flexible and proactive during high stress situations
Job Responsibility :
Personal care and support with showering, dressing, and grooming;
Assistance with domestic activities of cleaning, laundry and meal preparation;
Transportation to attend medical appointments and grocery shopping;
Attend social and leisure activities;
Support with the use of personal equipment and aids;
Monitoring Participant well-being and report on their progress;
Additional assistance and support according the participants support plan.
Behave in a respectful and courteous manner;
Work as part of a team and assist colleagues by providing support and advice;
Lead by example by providing direction when required
Comply with all policies and procedures
Adhere to administrative systems and work processes
Ensure the delivery of service provision meets Participant satisfaction
Meet all OHS training and compliance requirements
Actively contribute to ensuring a safe work environment
Comply with all safe working procedures and policies
