**Salary Sacrifice Details**

The **Care Coordinator** is responsible for providing advanced clinical support, leadership and coordination of clinical care for residents/clients with complex needs. You will provide mentorship and guidance for a care team of PCW's and Registered Nurses within our facility.
In this role you will:
- Oversee and co-ordinate the completion of baseline and required focus assessments of individual residents/client's needs and record findings within a Positive Wellbeing Model;
- Ensure that care plans are developed and reviewed based on the assessment findings of individual residents/client's needs, in accordance with the organisation's timelines;
- Co-ordinate the collection of accurate clinical/care data for timely submission of Aged Care Funding Instrument;
- Monitor that resident/client documentation is accurately reflective of care provisions and is in accordance with organisational requirements;
- Co-ordinate the care provision of external service providers;
- Assist in the review and maintenance of clinical programs and initiatives;
- Co-ordinate the collection of clinical indicator data in accordance with the organisation's reporting timelines.
- ** Part Time Care Co-ordinator Roster - 4 days per fortnight - includes weekends.**:
- ** 1st week - Tuesday and Friday / 2nd Week - Saturday and Sunday.**

You are often recognised for your highly developed problem-solving, conflict resolution, decision-making and negotiation skills.
As the Care Coordinator, ideally your experience and qualifications include:
- Current AHPRA registration as a Registered Nurse, with aged care experience;
- Experience in managing and leading teams in the aged care sector;
- Strong knowledge of the aged care accreditation system and compliance;
- Demonstrated knowledge of ACFI/AC-ANN;
- Experience in educating, mentoring and motivating staff;
- Superior communication skills both written and verbal;
- Possess or have the ability to obtain a National Police and NDIS Screening Check;
- National Personal Insolvency Check or the ability to acquire.
