Main Duties and Responsibilities
* Provide person‑centred care to help customers achieve their goals and aspirations
* Contribute information for the development of customer support plans
* Provide support to customers with personal care, living skills, transport, social activities, medication and other support as required
* Keep customer records up to date on a daily basis
* Develop and maintain professional relationships and networks with customers, families, caregivers and key stakeholders
* Seek out innovative ways to enhance the customer journey with support and guidance from our growing team
Requirements
* Completed Certificate III or IV in disability, individual support, aged care, or nursing
* Applicants with a valid licence and own vehicle (licence and comprehensive insurance) will be prioritised
* NDIS Worker Screening Check (or willingness to apply)
* At least 6 months experience in disability, aged care or nursing sector
* Proof of vaccination (COVID‑19)
* Working with Children Check (or willingness to apply)
