The Opportunity
We are currently seeking experienced Clinical Nurses to join our highly dedicated Referral and Care Support team based in Keswick. In this role you will be at the forefront of Referral And Care Support Team to provide leadership and support client centred onboarding experience and ongoing care support services.
The Role
Use your clinical knowledge, previous experience and leadership skills to Triage referrals and new or ongoing care needs requests to appropriate funding streams. Identify, provide support and seamless transitions of clients through services maintaining a client centred service. Provide clinical advice to current clients and staff that align with Best Care standard and Silverchain mission and values. Provide clinical support and oversight to the other members of the Referral and Care Support team and the Virtual Hospital Unit and its team of Enrolled Nurses
The roles Part Time by negotiation, rotating across 7 days, 365 days – Public Holidays included between the hours of 0700 – 2230 with multiple shift start times.
Key Responsibilities
* Utilise your leadership experience to provide clinical support across RDNS Silverchain services
* Maintaining contractual compliance and obligations
* This role progresses clinical governance and clinical practice across the organisation with responsibility for strategic clinical leadership and Clinical Governance advice across SA operations.
About You
* Current registration with AHPRA and a minimum 4 years post-graduate clinical experience
* National Police Clearance, NDIS worker clearance, Working with Children
* Out of the box thinking to get the best results for our clients
* Critical decision maker, able to make decision autonomously in complex situations
* Outstanding communication skills (written, verbal and listening) with a friendly and empathetic approach
* Computer literacy, including Comcare, Microsoft Office suite and Portals
* Ability to respond to dynamic clinical management situations
* Advanced problem solving, conflict resolution and negotiation skills
* Telephone triage and assessment skills
* Knowledge of telephone health advice, complex client management, funding programs, community services
