We are seeking a dedicated Nurse Practitioner to join our team in Tamworth, NSW.
About the Role
This is an exceptional opportunity to make a lasting impact during a transformative time in aged care. As a key member of our healthcare team, you will collaborate closely with Nurses, General Practitioners, Physiotherapists, Dietitians, Occupational Therapists, Social Workers, and other allied health professionals to deliver high-quality patient-centered care.
* You will be responsible for reducing avoidable hospital admissions through early intervention and comprehensive care, prescribing medications and providing specialist referrals as part of holistic, resident-focused treatment plans.
* Support wellness initiatives, including monitoring residents on wellness programs and following up in partnership with GPs.
* Lead clinical research by collecting, analysing, and interpreting wellbeing program data.
* Contribute to policy development and shape best-practice care models within the aged care sector.
Key Skills and Qualifications:
* Degree-qualified Registered Nurse or equivalent.
* Nurse Practitioner qualifications and experience.
* Current AHPRA registration.
* Ability to work independently and as part of a team.
* Excellent communication and interpersonal skills.
