About us:

Gladstone Region Aboriginal and Islander Community Controlled Health Service Limited ("GRAICCHS") trading as Nhulundu Health Service is an Aboriginal Community Controlled Health Service operating in Gladstone and the greater Central Queensland Region delivering a comprehensive range of primary health care, aged care and community support service functions.

GRAICCHS mission is that all Aboriginal and Torres Strait Islander Community members, and the community as a whole, have the right to access equitable and high-quality health care, regardless of culture, ethnicity or where they live.

About the position:

The position of Family Wellbeing Officer will work closely as part of a team with the Program Coordinator, Family Wellbeing Officers and Child Safety Officers in providing practical services and support:

* To build knowledge, strength and capacity of individuals and their families;
* To build individual capability to participate in and lead the coordination of their own supports and goals in improvement of relationships and/or family reunification or preservation;
* To support a positive cultural identity for all children through actions that enhance/encourage strong connections with kin, culture and country..

More specifically this role will focus on key activities, including but not limited to:

* Providing culturally appropriate social and family support services for clients, in line with the developed and agreed Family Care Plan:
* Support and assist clients with attending health and non-health appointments and including first and follow up appointments and attending Family Care Coordination appointments and other appointments, as required.
* Support and assist clients with non-health specific needs (such as access to Centrelink, employment, other psycho-social needs).
* Assist clients with transport services, prescriptions and PBS co-payments.
* Advocate for clients to address barriers.
* Develop and implement parental participation, education and strategies that lead to improvement in practical skills for family well-being.
* Support families to maintain links with their extended community members and kin connections.
* Providing practical services and supports to build knowledge, strength and capacity of individuals and their families to provide the necessary environment to optimise the trajectory of health and wellbeing of children.
* Delivering support in a manner which ensures meaningful engagement of the person in all decision making, with a bias toward building individual capability to participate in and lead the coordination of their own supports. At times, there may be a requirement for the role to advocate on behalf of the client (and their family).
* Keeping abreast of and updating service options and promoting available services and supports relevant to the clients' plan.

This position is based in Gladstone and outreach across the Biloela region

About you:

To be successful in this position, we are seeking an individual which possess' the following:

* Certificate III in Youth Work, and/or Aboriginal/Torres Strait Islander Primary Health and/or equivalent relevant experience – preferred.
* Demonstrated experience working with vulnerable people and their families, including individual youth work and community engagement/development programs.
* Some work outside of normal hours of duty (including on call) may be required as well as some travel across the region
* Current C Class Drivers Licence (Qld) – essential
* Satisfactory Criminal History Check - essential
* Current Blue Card (working with children and young people) or willingness to apply for a Blue Card – essential
* Ability to meet Child Safety and Personal History Screening (LCS2) requirements - essential
* First Aid & CPR – desirable
