This role requires you to provide tailored care including personal support within the residential home.
About the role: As the Manager for complex behaviour cottages supporting people living with dementia, you will be a core member of the Hammondville (Southwood) management team and work with a wide variety of people including the Residential Care Operations team, Local Health District, RNs, Carers, residents, resident representatives, and the multi-disciplinary health team to ensure resident outcomes are met.
Essence of the role: • Advocate for residents to ensure their care is person-centred and focused on their strengths, capacities and promotes their well-being at every stage.
• Facilitation of clinical care delivery by the Registered Nurses and Specialised Carers • Hands-on modelling, coaching and mentoring in clinical and complex behaviour management areas and processes • Consultative in case management and clinical decision making • Clinical oversight and professional competence as a Registered Nurse with experience in complex dementia-specific care • Support and investigate clinical/resident incidents as required • Communicate effectively with all stakeholders, both within HammondCare and external partners to see the best outcomes for the residents.
To be successful in this role, you will have: • Demonstrated ability to support complex behaviours and psychological symptoms of dementia, motivate and develop a team and effectively work with other staff.• Strong negotiation and conflict resolution skills.
• Significant experience operating as a consultant or specialist nurse preferably within Aged Care • Demonstrated ability to support complex behaviours and psychological symptoms of dementia, motivate and develop a team and effectively work with other staff.
• Solid leadership and influencing skills.
• Excellent analytical skills.
• Strong administrative and computer skills.
• Tertiary qualifications in specialised dementia and/or mental health service nursing for older people.
