Darling Downs and West Moreton PHN is a not for profit organisation that works at a regional level to achieve improved whole of system health care.
Our key objectives are to:
increase the efficiency and effectiveness of health services for patients—particularly those at risk of poor health outcomes, and
improve coordination of care to ensure patients receive the right care, in the right place, at the right time.
Our PHN achieves this by working with local health care stakeholders and community to:
fund programs and services based on local health needs;
support primary health care professionals, like GPs, general practice staff and allied health professionals, to improve the care they provide to their patients; and
support the development of a local sustainable health care workforce.
About the Role
The role will build capability across the local system by embedding neuro-knowledge and a shared understanding or what works and why in supporting children and families. Through coaching, learning and development, the role will strengthen the skills confidence, and practice of communities, families, practitioners and organisations working with infants, children, young people and caregivers. Contributing to better outcomes and greater more cohesive approach to this area. Queensland Kids Partnership will facilitate a community of Practice of all Capability Champions and organisations across Queensland offering peer to peer support and will facilitate the evaluation of the project.
The role will take direct responsibility for place-based commissioned with in the West Moreton corridor, working across health, social care, community organisations, and families to connect and coordinate the delivery of the Early Years Strategy at a local level. Hosted by Darling Downs and West Moreton PHN but embedded within the community, the role will bridge connection across partners and the broader system to ensure children and families receive consistent, responsive, and effective care and support. Facilitate and maintain strong, productive contractual relationships with commissioned service providers to ensure optimal delivery of program outcomes for the community.
Foster a strong partnership approach that embeds the voice of the community throughout the delivery of the two components is crucial to the success of the project.
Key Responsibilities
Build workforce and organisational capability and confidence to embed and sustain neuro-informed policy and practice, underpinned by relational ways of working.
Provide organisational and team coaching and implementation support.
Enhance cross-sector collaboration by facilitating a shared language and understanding of core child development knowledge: brain development and neurodiversity, trauma and stress, healing and resilience, agency and learning.
Strengthen innovative and holistic approaches to protective and developmental factors embedded in the NEST Framework, Australia's Child Development and Wellbeing Framework, and The Resilience Scale.
Develop a local health needs assessment, map existing services, identify gaps, and work with families, communities and providers to develop and manage commissioning plans that reflect identified needs.
Utilise appropriate governance mechanisms throughout the commissioning cycle, ensuring evidence-informed planning, procurement, contracting, implementation, monitoring and evaluation.
Embed and continuously improve commissioning frameworks, including culturally responsive approaches that improve accessibility and effectiveness.
Contribute to the development and implementation of place-based strategies to improve health outcomes in response to regional priorities
Work with the Health Intelligence team to develop a local health needs assessment, map existing services, identify gaps, and work with families, communities and providers to develop and manage commissioning plans that reflect identified needs.
Build and maintain collaborative relationships with key stakeholders including other PHN’s, Hospital and Health services, and local providers to endure commissioned services are integrated, address priority population health needs and deliver better community outcomes.
About You
You are an accomplished senior leader with experience in health, human services, community services or public health planning.
You will bring:
Relevant tertiary qualifications and experience in health, human services, or relevant related field
High level knowledge and understanding of contemporary evidence-based service delivery models, planning, strategies, and guiding policy documents across the Early Years priority.
Understanding of and commitment to the principles and practices that support health equity and demonstrated knowledge of the primary health care and community services sectors
Demonstrated ability to partner and collaborate with a wide network of individuals and organisations to achieve meaningful outcomes and build successful relationships and partnerships with key stakeholders both internally and externally.
Excellent written and verbal communication and interpersonal skills and the ability to engage well at all levels and exceptional time management skills with the ability to handle multiple time critical projects simultaneously
Exceptional project management skills with demonstrated experience managing and evaluating complex projects with a diverse range of stakeholders
Demonstrated ability to lead a critical parcel of work as part of a broader team
Strong analytical skills, with the ability to synthesize, interpret and translate complex qualitative and quantitative data into accessible briefs, concepts, plans and reports
