Job Description:
The Registered Nurse position within the Assertive Case Management (ACM) program is part of a multi-disciplinary team that provides intensive care coordination for consumers with chronic and severe substance use issues.
Main Responsibilities:
* Work collaboratively with clients to develop and implement personalized plans aimed at stabilizing their condition and reducing substance use.
* Provide comprehensive support to address complex needs, including cognitive impairment, physical and mental health, nutrition, and social functioning.
* Establish prevention strategies to reduce the risk of relapse and promote long-term recovery.
