This is a rewarding opportunity to provide specialised, patient-centred care to individuals managing chronic heart failure and contribute to improving their quality of life and long-term outcomes.As a Heart Failure Registered Nurse, you will play a key role in the delivery of care coordination, patient education and chronic disease management for individuals living with heart failure.
