Job Title: Allied Health Professional - Transition Care
At Barwon Health, we are committed to delivering high-quality resident-centred care. As an Allied Health Professional in our Transition Care Program, you will play a key role in supporting older adults following hospital discharge.
The program provides support for clients with tailored care plans and ongoing coordination. Our multidisciplinary team includes nurses, allied health professionals, and care coordinators working together to deliver comprehensive care.
You will contribute to the care of clients by delivering client-centred, evidence-based services under the supervision of the Allied Health and Care Coordination teams. You will also support nursing staff with direct care functions and assist with non-clinical duties to ensure seamless service delivery within the Transition Care Program.
About the Role:
* Contribute to the development and implementation of individualized care plans for clients
* Deliver evidence-based interventions to support clients' physical, emotional, and social needs
* Collaborate with the multidisciplinary team to coordinate client care and services
* Provide education and support to clients and their families on managing chronic conditions and promoting healthy lifestyles
* Assist with administrative tasks and reporting as required
