About the Role:
We are seeking a highly skilled and dedicated individual to join our team as a Dementia Support Coach. This role will provide an opportunity to make a meaningful impact every day by supporting people living with dementia as they transition from hospital to home or into residential care.
Duties and Responsibilities:
* Provide on-the-ground support and implement relationship-based strategies for clients, carers and care staff.
* Model and coach best-practice approaches to help carers and aged care staff understand and support people living with dementia.
* Implement recommendation plans with guidance from Team Leaders, Consultants and Senior Professionals.
* Facilitate and support the trial and implementation of brokerage items for clients and carers.
* Collaborate with peers to build capability across aged care teams and support quality, person-centred care.
Requirements:
* Strong organisational and time-management skills in a busy work environment.
* Ability to work independently and collaboratively.
* A commitment to excellent customer service and constructive conflict resolution.
* A current driver's licence and safe driving record.
* Willingness to travel intrastate and interstate for up to five days at a time.
* Flexibility to work varying hours based on client and carer needs.
* Qualifications in a relevant field such as Allied Health, Diversional Therapy, Aged Care or Individual Support.
