Case Manager – Recovery & Support
A State Government Agency is seeking a Case Manager to manage a portfolio of injured individuals and support recovery and return-to-life outcomes through active engagement.
Key Responsibilities:
* Coordinate recovery plans and manage complex cases
* Provide empathetic support and proactive communication with individuals
* Engage with support networks, service providers, and internal stakeholders
* Identify services and programs that improve recovery and social outcomes
About the Role:
This is an exciting opportunity for a motivated and experienced Case Manager to join our team. The successful candidate will have a strong understanding of recovery models, support services, and return-to-work strategies.
Requirements:
* 2+ years' case management experience in personal injury, health, or disability
* Comfortable supporting sensitive and complex cases
* Tertiary qualifications in allied health, rehabilitation, or related fields are highly desirable
* Previous experience with relevant software is highly regarded
