About the role: The position requires the successful RN to provide care to patients in the community following a hospital stay by providing an efficient and effective Transitional Aged Care Program (TACP) Service including timely response to accepting referrals for intake assessments in the hospital, case management, assisting the patient to regain functionality & mobility and finalise ongoing care arrangements. About you: The successful applicant will be a motivated Registered Nurse who can take initiative when needed whilst out in the community. You will provide support and leadership to your fellow RN's, EEN's and Allied Health professionals in your team, in both Forster and Taree. You will possess strong interpersonal and communication skills, have the ability to consult with a multi-disciplinary team and be able to work effectively to deliver a professional service to the community. Requirements: Current registration as a Registered Nurse with AHPRA. Unrestricted NSW Drivers Licence with the ability and willingness to visit clients in their home across the Lower Mid North Coast of NSW.
